Located just off I-235 on Des Moines' west-side, our location is convenient for residents, families, and visitors.

From I-235, take the 42nd Street exit.
Follow 42nd Street north one block to Chamberlain Avenue.
Turn west (left) on Chamberlain Avenue and follow three blocks to Polk Boulevard.
The Life Center is located just beyond the intersection of Chamberlain Avenue and Polk Boulevard.
Our main entrance is on the west-side of the building.
