What to expect when the next step in recovery includes short-term rehabilitation
Leaving the hospital is an important milestone, but for many older adults, recovery isn’t complete. After an illness, surgery, injury, or extended hospital stay, additional therapy and nursing support may be needed before returning home.
Why Rehabilitation May Be Recommended
A hospital care team may recommend short-term rehabilitation when someone is medically ready to leave the hospital but isn’t quite ready to safely manage everyday activities at home. The goal is to provide the right level of support during recovery while helping each person become as independent as possible.
Physical and Occupational Therapy Support
The Life Center offers both physical therapy (PT) and occupational therapy (OT) to support individual recovery goals.
Physical therapy can focus on areas such as strength, balance, walking, and mobility. Occupational therapy helps patients work on the everyday skills they need to function more independently, including dressing, bathing, and other daily activities.
Because every recovery is different, therapy is based on each person’s individual needs and abilities.
Making the Transition Easier
The transition from the hospital to rehabilitation can happen quickly, and families may suddenly have a lot to coordinate.
The Life Center’s Admissions Team works with patients, families, and healthcare providers to help make the transition as smooth as possible. From answering questions and coordinating admission details to helping families understand what to expect, our team is here to help throughout the process.
Before arriving, we’ll also help you understand what information is needed, what to bring, and what you can expect during your stay.
Care That Goes Beyond Therapy
Recovery isn’t limited to time spent in therapy. During a short-term rehabilitation stay at the Life Center, patients also have access to skilled nursing care and assistance with daily needs.
Our team works together to support each person’s recovery, monitor progress, and adjust care as needs change.
Planning for What Comes Next
From the beginning, rehabilitation is focused on helping each person work toward their next step.
For some patients, that means returning home independently. Others may need additional assistance, continued therapy, or another level of care. The care team works with patients and families to understand ongoing needs and prepare for the transition ahead.
